Mission Statement:

The mission of Oasis Luxury Med Spa is to provide superior medical-grade aesthetic services while educating our clients and engaging them in decisions regarding the health of their skin. Our staff strives to remain at the forefront of education, technology, and research in aesthetics. We will listen to each client's needs and always make him or her feel respected and embraced while offering the highest quality treatments. We will do this in an environment that is relaxing, pampering, friendly and professional.

Credit Card on File

All clients must have a valid credit card on file in order to book an appointment. 

Late / Reschedule / Cancel / No-Show

LATE: If you arrive to your appointment late your treatment will be shortened. If you arrive more than 15-minutes late your appointment will be cancelled, and you'll be charged the CANCELLATION FEE (see below).

RESCHEDULE OR CANCEL: Must be done 48-HOURS OR MORE before your scheduled appointment. Failing to cancel or reschedule 48-hours before your booking will result in a $100 CANCELLATION FEE being charged to your credit card on file. If the Cancellation Fee cannot be processed you will no longer have access to booking with Oasis Luxury Med Spa until the fee is paid. Please contact me directly for exceptional circumstances and we can make other arrangements.

NO-SHOW: Will result in a $100 NO SHOW FEE being charged to your credit card on file.  If the No Show Fee cannot be processed you will no longer have access to booking with Oasis Luxury Med Spa  until the fee is paid. Please contact me directly for exceptional circumstances and we can make other arrangements.

SICK: 

We consistently provide an environment for our staff and for our clients that is health aware, safe, clean, and disease free. We ask clients to please cancel and reschedule their appointment if they are not well and have any kind of contagious viral or bacterial infection. Coming into Oasis Luxury Med Spa when you are unwell puts myself and other clients at risk of exposure to also getting infected. Should you be sick or have a temperate of 100.4 or higher you will be asked to leave. If you need to cancel your appointment within less than 48-hours prior to your booking, please contact us directly at info@oasisluxurymedspa.com or by phone at 832-295-3086.

Arrive 10 minutes prior to your appointment to fill out forms needed for your visit. 

Treatment Forms | Medical History

Skin Consultation and Treatment Consent Forms must be filled out by all clients. This information is collected in order to ensure I have a clear understanding of your medical history, medications, active topicals being used, allergies, recent surgeries, etc. This will enable me to clearly identify any contraindications to treatments or products. The goal is to always ensure you are an optimal candidate for treatment(s) and to identify if a treatment needs to be altered to suit your needs. Your health and safety is my top priority - therefore it is mandatory to fill out all required forms and to be 100% honest in all answers. All medical information will be kept confidential, and I abide by all privacy laws.  

Minors

Clients under the age of 18 must present written permission from their parents or guardian prior to their first service, or have their parents present at initial consultation. Clients under 15 years of age must have a parent or guardian present at all times.

Phones: 

Phones or earphones are not allowed to be used in the treatment rooms and must be kept on silent throughout your consultation and/or treatment. 

Method of Payment | Financing

We accept Visa, MasterCard, AMEX, Cash and Care Credit. Taxes and gratuities are not included in the service price.

Gratuities

Gratuities are entirely at your discretion and are not included in the price of treatments but often range from 18% to 20% in the spa industry. Please remember that gratuities are much appreciated for time and services rendered but in no way expected. 

Pricing

Treatment and product pricing is subject to change at any time t. 

Promotions

Specials/Promo/Sales can not be used or combined with any other offer, promotions or third-party gift certificates and online vouchers. (You cannot use any reward cards or points for any Neurotoxins such as Botox or Jeuveau and/or Dermal Fillers (Revanesse, radiesse, Juvaderm) promotions.

Returns & Exchanges

All online shop and in-store purchases are final sale.

Should you experience reaction due to skincare, you must contact info@oasisluxurymedspa.com within 3-days of receiving your item with details of your reaction and what other skincare you are using with it. Your email will be reviewed to assess what may be causing a reaction, but they are final sale.

All treatments are non-refundable or transferable.

Series of Treatments Return Policy: Oasis Luxury Med Spa is unable to refund payment of treatment series once it has been purchased. However, Oasis Luxury Med Spa will provide a credit of equal value for future use with in one- year. (EXPIRES AFTER 12 MONTHS)

You are fully responsible for your own health throughout any treatment and/or stay with us and therefore it is your responsibility to ensure that you are medically fit to undergo any treatment. You are hereby advised to seek advice from your doctor before undergoing any treatment with Oasis Luxury Med Spa.

The medical aesthetician, laser tech, providers. and Oasis Luxury Med Spa may reserve the right to refuse service to anyone for inappropriate behavior, illness listed as contraindications for services or as deemed necessary. The client also has the right to stop the treatments at any time. The medical aesthetician, laser tech, providers. and Oasis Luxury Med Spa assumes no responsibility for any disclosed or undisclosed illness or personal injury. Results might not meet your expectations or the goals that have been established. It is explained that no guarantees or assurance is made regarding the procedure requested and authorized. Additionally, you understand that multiple treatments will most likely be needed to achieve desired end point and that individual results may vary.

Oasis Luxury Med Spa has the right to cancel or reschedule a treatment without notice. We have the right to change your service without notice. 

By calling, emailing info@oasisluxurymedspa.com or scheduling your appointment online or directly, you are indicating that you understand and accept these terms and conditions outlined above.
